    Mark, I once heard a lecture by Michael Dyber, who makes fabulous
    faceted stones with tubes drilled in them.  He was not willing to
    share this particular technique with us but, having heard how he does
    other things, my guess at his tool  would be a toothpick or other
    wood shard chucked in a rotary handpiece.
